Do I need a doctor’s referral?

Referrals are required based upon your insurance. Call us to verify.

What is your new patient procedure?

When coming into our office, insurance card and ID are required. Forms have to be filled out and upon completion, you are taken to the examination room, where Dr. Gutierrez evaluates your case and discusses treatment plans.
