Do I need a referral?
Referrals are required based upon your insurance. Call us to verify.
No Insurance? No problem!

Initial Visit / Exam
Please be advised, if your last visit was over a year prior to your new appointment, an Initial Exam fee will be charged.

Every follow up visit after.
Important Office/Appointment Guidelines:
- We do not accept New Patient WALK INS. All New patients MUST make an appointment in advance.
- We do NOT accept WALK INS past 5PM.
- We DO accept same day appointments for both returning and new patients. Call us in the AM to check for same day appointments.
- As per our office guidelines, patients who are 15mins late to their appointment will be asked to reschedule.
